Insurance Policy & Payment Responsibility
Marino Wellness & Medspa does not accept insurance, including Medicare, Medicaid, managed care plans, or private health insurance, for any services, supplements, vitamins, or pharmaceutical products. All services are self-pay, and clients are solely responsible for payment regardless of any third-party coverage.
Service Disclaimer
Marino Wellness & Medspa does not diagnose or treat any illness, disease, or medical condition. By receiving our services, you acknowledge and affirm that you are not seeking medical diagnosis or treatment. Our staff does not provide screening, monitoring, or therapeutic interventions for medical conditions.
Consent for Use and Disclosure of PHI
By consenting to our services, you authorize Marino Wellness & Medspa to use and disclose your Protected Health Information (PHI) for treatment, payment, and healthcare operations (TPO), as described in our HIPAA Notice of Privacy Practices. You may request restrictions on how your PHI is used or disclosed, and revoke consent in writing at any time, except when already acted upon.
Communication Preferences
With your consent, we may contact you by phone, voicemail, mail, or email for scheduling, billing, or clinical communication purposes. These communications will be marked “Personal and Confidential” when appropriate.
Informed Consent & Acknowledgment of Risks
By receiving services from Marino Wellness & Medspa, you understand that IV therapy, vitamin and supplement infusions, and other wellness treatments carry certain risks, including but not limited to:
- Injection site reactions (bruising, swelling, pain)
- Infection or inflammation
- Allergic reactions or medication interactions
- Dizziness or fainting
- Air embolism or fluid overload
You confirm that you have disclosed relevant health history, medications, and conditions. Marino Wellness & Medspa is not responsible for adverse outcomes arising from nondisclosure or from client-provided substances.
Drug-Free Declaration
You affirm that you are not under the influence of illegal substances or controlled drugs at the time of service and are not undergoing detoxification from such substances.
Emergency Protocol
In the event of a medical emergency, please call 911 or go to the nearest emergency room. Marino Wellness & Medspa does not provide emergency medical services.
Final Acknowledgment
By signing or accepting these terms electronically, you confirm that:
- You have read and understood the information above.
- No guarantees have been made regarding outcomes or results.
- All your questions have been answered to your satisfaction.
- You voluntarily assume all risks associated with your participation in our services.
This agreement is binding upon acceptance and remains in effect for all services rendered by Marino Wellness & Medspa.
